What documents should I have ready before completing the intake form?

The more detail you can provide, the more useful your response will be. Helpful items include: the person's diagnosis or condition, any current benefit amounts, recent letters from SSA or DFCS, and any upcoming deadlines. You can also upload documents directly through the intake form — up to 5 files, 10 MB each.
