Georgia Medicaid Waiver Document Checklist

Use this checklist to prepare for NOW/COMP waiver applications, Katie Beckett (TEFRA Medicaid), and GAPP in Georgia. Documents required vary by program — use the sections below for the program(s) that apply to your family.

NOW / COMP Waiver (DBHDD)

IDD diagnosis from a qualified professional

Must be from a licensed psychologist, physician, or other qualified evaluator; diagnosis must meet Georgia's definition of intellectual or developmental disability

Current Georgia Medicaid eligibility confirmation

Medicaid ID card or letter from DFCS confirming active Medicaid coverage

DBHDD level-of-care assessment

Completed by a DBHDD-authorized assessor after you are placed on the waitlist

Person-centered support plan

Developed with your Support Coordinator once you reach the top of the waitlist

Proof of Georgia residency

Utility bill, lease, or official mail with current address

Social Security number for the individual with the disability

Waitlist Tip

The NOW/COMP waitlist currently has over 7,000 individuals and wait times of 7–12 years. Get on the list as early as possible — contact DBHDD at 1-888-275-7229 or visit dbhdd.georgia.gov to request a waitlist application. You must be on the waitlist before you can receive waiver services.

Katie Beckett (TEFRA Medicaid) — Children Under 19

Child's medical records documenting the disabling condition

Must show the child requires an institutional level of care (hospital, nursing facility, or ICF/IID level)

Physician statement documenting institutional level of care

Letter from the child's primary physician or specialist confirming the level-of-care requirement

Child's Social Security number

Proof of Georgia residency for the child

Utility bill, lease, or school enrollment records

Child's birth certificate

Documentation that the child can be safely served at home

Physician or care team statement confirming home-based care is appropriate

Katie Beckett Tip

Only the child's own income and resources are counted for Katie Beckett — not the parents'. This makes it available to families who would not qualify for regular Medicaid. Applications are submitted through DFCS. This program is significantly underutilized in Georgia.

GAPP (Georgia Pediatric Program)

Active Georgia Medicaid ID

Physician orders for private duty nursing or personal care services

Nursing assessment completed by a registered nurse

Documents the child's medical needs and required level of nursing care

Documentation of medical necessity

Records showing the child requires skilled nursing care in the home
