SSI / SSDI Document Checklist — Georgia Benefits Support

Use this checklist to gather everything you need before applying for Supplemental Security Income (SSI) or Social Security Disability Insurance (SSDI) in Georgia. Having these documents ready can significantly reduce processing delays.

Identity & Citizenship

Birth certificate or proof of age

Certified copy preferred; hospital record or baptismal certificate may be accepted

Social Security card

Or proof of Social Security number (SSN)

Proof of U.S. citizenship or lawful alien status

Passport, naturalization certificate, or immigration documents

Government-issued photo ID

Driver's license, state ID, or passport

Medical Records

Names, addresses, and phone numbers of all treating physicians, hospitals, and clinics

Include specialists, therapists, and any facility where the person has been treated

Recent treatment notes from the past 12 months

Especially from specialists relevant to the disabling condition

Hospitalization records (if applicable)

Psychological or neuropsychological evaluation reports

Especially important for intellectual disabilities, autism, or mental health conditions

Functional assessments documenting what the person can and cannot do

IEP or school evaluation records (for children or young adults)

Includes eligibility reports, re-evaluations, and progress notes

List of all current medications and dosages

Financial Information (SSI Only)

Bank statements for the past 3 months (all accounts)

SSI has a $2,000 resource limit for individuals; $3,000 for couples

Proof of any other income (wages, pension, child support, etc.)

Proof of living situation

Lease, mortgage statement, or utility bills in your name

Documentation of any property owned (other than primary home and one vehicle)

Work History (SSDI Only)

Complete work history for the past 15 years

Employer names, addresses, dates of employment, and job duties

W-2 forms or tax returns for the past 2 years

Most recent pay stubs (if still working)

Military discharge papers (DD-214) if applicable

Georgia-Specific Tip

Georgia Disability Determination Services (DDS) in Atlanta handles the medical review for all Georgia SSI and SSDI applications. If DDS requests a consultative examination (CE), attend it — missing a CE appointment is one of the most common reasons for denial. You will not be charged for the examination.
